Wayne Saltsman, MD, PhD, CMD, is board-certified in Geriatric Medicine, Internal Medicine and Hospice and Palliative Care Medicine. He is the Chief Medical Officer for Continuing Care and the division Chair for Geriatric Medicine for Lahey Health in Burlington, MA. He is an elected board member for AMDA - The Society for PALTC Medicine, as well as the current chair of the Society's Transitions of Care Committee.
