Physicians, advance practice providers, social workers, clergy, nurses, psychology and psychiatry providers would benefit from learning the skills taught in this course.
Learning Objectives:
At the completion of this session, learners will be able to:
Discuss the complexities of determining prognosis for nursing home patients
Understand how to utilize different mortality scales such as the Mitchell Mortality Scale and eprognosis to get an estimate of prognosis
Define best practices for the interdisciplinary team to work together to improve prognosis of nursing home residents, or adjust goals of care if improvement is not possible.
Teach communication strategies to use with family or caregivers to discuss prognostic uncertainty
